Cartilage “Clew” Lesion: Report of a Severe Case of Cam-Type Hip Impingement after Slipped Capital Femoral Epiphysis

The purpose of this case report is to present a severe example of cam-type hip impingement following a slipping capital femoral epiphysis (SCFE), in which a periosteal sleeve rolled over itself, generating a clew of cartilage, which was discovered during a hip arthroscopy. In a 16-year-old female patient, this underlines the severity and relationship between SCFE and hip impingement, resulting in functional impairment and early onset of osteoarthritis. A total thickness detachment of an enormous section of femoral cartilage was characterised by the cartilage clew lesion, which appeared as a big flap of cartilage that folded in on itself. We believe that the slid epiphysis was large enough to cause a fracture in the femoral head’s subchondral bone during abduction/flexion and internal rotation. Because the mechanical problem persisted, the femoral cartilage detached, resulting in an extended flap that began to fold up on itself. Despite the fact that hip arthroscopy successfully managed this patient’s hip discomfort at one year follow-up, the likelihood of this young woman needing a hip replacement in the near future is still significant, highlighting the necessity of SCFE diagnosis and treatment.
